If your dentist has billed for the procedure (which would be normal), than the endodontist will need to bill you for more of the procedure cost, and possibly write more narrative to the insurance carrier. Files do separate inside canals from time to time. It is a fact of life. Most times, they separate after the canal has been cleaned well, and sealing around it works just fine. Sometimes, a root will have strange anatomy with extra curves and twists which do not show up on the x-ray image.
