Hello!

You are being invited to participate in a project designed to increase
understanding of chronic pain. Specifically, we are conducting a
study on persons with chronic pain who are involved in online support
groups. We are inviting participants (age 18 and older) who have been
and are currently experiencing a recurrent or “nonremitting” chronic
pain condition for at least 3 months to complete an online survey. It
is anticipated that this survey will take 45-60 minutes of your time
to complete.

Participants who complete the survey may elect also to be entered into
a drawing to win one of twelve $50 VISA gift cards. We expect to have
approximately 600 people complete the survey. If everyone enters the
drawing your odds of winning one of the gift cards would be 1 in 50.

members of your support group so that they may have the opportunity to
assist us in gathering information about chronic pain and online
support groups.

This study has been approved by Spalding University’s Research Ethics
Committee in support of dissertation requirements for the completion
of a doctorate of psychology (Psy.D.) degree. If you have any
