Objective
To investigate the possible relationships between plasma homocysteine
levels and thrombotic events in a select population of rheumatoid
arthritis (RA) patients with or without antiphospholipid (aPL)
antibody positivity, Methods 168 female RA patients attending the
Extra-articular Involvement RA Clinic of University of Genova and 72
female subjects matched for age and vascular diseases as controls were
included in the study. 30 of the RA patients showed aPL antibody
positivity and 138 aPL antibody negativity on the basis of the
concomitant presence or absence of high concentrations of
anticardiolipin (aCL) antibodies or the presence of lupus
anticoagulant (LA). All subjects were evaluated for plasma
homocysteine concentrations and for the occurrence of thrombotic
events. Results Twenty-five RA patients and 5 controls reported a
history of thrombotic events. Eleven and 5 of RA patients were found
to have been previously affected by venous or arterial thrombosis,
respectively. Plasma levels of homocysteine in aPL antibody positive
patients with thrombosis were found to be significantly higher than in
aPL antibody positive RA patients without thrombosis (p < 0.001). When
RA patients with thromboses were analyzed, a significant increase of
plasma homocysteine levels was found in aPL antibody-positive RA
patients versus aPL antibody-negative RA patients (p < 0.04) and
versus related controls (p < 0.003). Conclusions The association
observed between aPL antibody positivity and high levels of plasma
homocysteine in RA patients may represent a possible risk factor for
thrombotic events. Therefore, it is suggested that
hyperhomocysteinemia might be involved in the vascular-related
mortality observed in RA patients with a history of thrombosis.

Diabetes Mellitus Does Not Increase Thrombogenicity of the Arterial
Vessel Wall in Atherosclerotic LDLr-/- Mice

Background: Diabetes mellitus is associated with an increased risk
for acute arterial thrombosis. We evaluated the effect of
hyperglycemia on expression of proinflammatory and prothrombotic
genes, as well as on the prothrombotic potential of the vessel wall in
LDLr-/- mice.

Materials and methods: LDLr-/- mice were divided in three groups:
mice on control chow (n= 10), mice on western-type diet (n= 8) and
mice on western-type diet in combination with streptozotocin induced
diabetes (n= 9). The extent of atherosclerosis was assessed at the
aortic sinus. Vascular gene expression was measured by rt-PCR. Ferric
chloride-induced thrombus formation was evaluated in the carotid
artery.

Results: Diabetic mice possessed significantly higher glucose and
cholesterol levels than control mice. In addition, western-type diet
resulted in diabetic mice in 140% larger mean atherosclerotic lesion
size compared to control mice, (P= 0.066). Lesion size correlated to
both glucose (r2= 0.585; P < 0.001) and cholesterol levels (r2= 0.563;
P < 0.001). Tissue factor and p-selectin mRNA levels in the arterial
wall increased 20% in mice on western diet compared to control fed
mice (P < 0.05). No further augmentation was observed in diabetic mice
on western-type diet. Surprisingly, no difference was observed in time
to thrombus formation between mice with and without western-type diet
and with or without diabetes.

Conclusions: Although induction of diabetes, leading to hyperglycemia
and hypercholesterolemia, was associated with larger plaque size,
changes in vascular gene expression or thrombus formation were not
observed. This suggests that the increased risk for atherothrombotic
complications in diabetes mellitus is probably more related to
accelerated atherosclerosis than to diabetes specific prothrombotic
changes in the vessel wall.

Treatment:
The objective of treatment is to reduce the high blood viscosity
(thickness of the blood) due to the increased red blood cell mass and
to prevent hemorrhage and thrombosis.

Phlebotomy is one method used to reduce the high blood viscosity. In
phlebotomy, 1 unit (pint) of blood is removed weekly until the
hematocrit is less than 45, then phlebotomy is continued as necessary.

Occasionally, chemotherapy may be given to suppress the bone marrow.

The use of anti-platelet therapy (such as aspirin) is controversial
because it may cause gastric bleeding.

Allopurinol is given for hyperuricemia (gout).
