I'll suggest the more incline the better. I used 18 inches of incline for a number of years of late I been getting by with 12 inches and sometimes sleeping on the level. I dimly recall Billy Boy saying he uses 9 inches of incline and his bed has a top quality foam mattress as well. Innerspring mattresses don't take kindly to being inclined and thus cause bad back pain, IMO. GRJ has antiinflammatory properties. While it didn't seem to help my GERD in the past, I do use now and then as Ginger root tea as it helps with this late middle aged man with some of aches and pains. Melatonin may also be helpful. Understand I feel sleeping on an incline is really important. Night time reflux has the longer residency time especially if one is sleeping level. Thus it has more time to "burn" thru the mucus layer that protects the esophageal lumen lining. As Howard says bile salts can be a problem even if stomach acidity is reduce by meds. In my case, taking 4 or 5 capsules of betaine HCL and pepsin with larger fatty meals of the day and this makes a profound difference. This is alternative medicine. As the PPI meds helped some, It took me along time to try it. I am not saying this will work for you, rather that it works for me. I hate the the PPI meds as while they are useful, they got to be quite toxic with a longer periods of use. I've been off of the PPI meds for 3 3/4 years by sleeping on a high incline and using the plenty of the digestive supplement (10 grain size) betaine HCL with pepsin. I used the PPI meds, the H-2 acid reducers and I had a fundo that went leaky with time. I spent 1000's of dollars and waste a decade of my life due to GERD and the conventional approach. The only part of the conventional approach I suspect as having value are the fundos, I had mine 11 years ago.
