From: Ken on
Good Morning-

I posted awhile back about my wife being diagnosed with GERD and her
heart disease history.
After two months of testing they have come to the conclusion she has
GERD.

Her concern is the severe pain in her lower jaw anytime she exerts
herself. Bending over and wiping the floor, doing laundry, a brisk
walk.

She is very upset at the world right now she will not go to Mayo
Clinic, she is tired of the poking and prodding.

She is taking Previsad <sp> and 2x a day and 75mg Zantac 2x a day. She
is eating small meals 6 times a day, no diet coke (her only vise along
with dark chocolate which she has given up as well).

She sleeps on an incline and can't sleep on her side.

Will this go away? Can she exercise with no lower jaw pain?

It is like a double edged sword.
Her 5 bypass in 2002 11-29 -07 will be 5 years. Her heart is in
excellent health. She did not have a heart attack. Under this last
battery of tests her heart, veins, arteries, and bypasses are 100%
clear and healthy, from head to toe. As posted earlier all things
related to GERD check ok i.e. gallbladder.

Today you need to be your own health advocate, her GP is a delegater
(not a word), I use the word because she just sends my wife to
specialists and really has not made a decision on her own experience
as a doctor in my opinion. Which is not all bad.

We had a conference at my request with an Endo, a Gastro, and two
Cardiologists, a Neurologist (my friend) and her GP.
Conclusion: You have severe GERD could take 8 months to a year to
subside, it may never if you decide to not follow the advise given.

My wife has done cardio exercise for at least 1 hour every day for the
last 5 years minus the last two months. She has no energy and is
feeling very depressed.

I am frustrated for her.
Thank you for letting me vent.

Has she seen her dentist yet?

As in my earlier reply, jaw pain has nothing to do with GERD. It is a
separate condition. She should discuss TMJD with her dentist and, if
necessary, go and see an orthodontist.
http://www.nlm.nih.gov/medlineplus/ency/article/001227.htm

If she has TMJD then a simple mouth splint may be all that is needed to get
rid of the pain. http://www.jawjointpainrelief.com/TMJ_appliance.asp
http://www.dentalgentlecare.com/new_page_23.htm

I wear my mouthguard at night and it is fitted to my lower jaw and not upper
and lower - the cheaper version. I do have breakthrough TMJD pain at the
moment, but I have made an appointment with my dentist and will probably
have my teeth ground down and a new mouthguard made.

Has she had ambulatory pH testing? That test is the only method of
definitevely diagnosing GERD. During the 24 or 48 hours that the esophageal
pH is being measured, the patient records the specific activity he or she is
doing when the symptoms occur. When she has the jaw pain while excercising,
it can be determined whether or not that is associated with gastric contents
in her esophagus.

Unfortunately, reflux symptoms may not subside even if the patient does
everything to follow the advice given. She is already on a very aggressive
anti-acid medication regimen.

Vanny-

She has been to her dentist, no one can explain the jaw pain from
exertion. Tomorrow morning back to the GP for questions on GERD.

Thank you to all who responded.

Ken
From: Vanny on
Ken,

If you look at the references I gave you in my first response to your first
e-mail - jaw pain on exertion is a classical symptom of TMJD. If the dentist
does not know what TMJD is then you should get a referral to an orthodontist
or see another dentist.

It would be a shame if she ends up having months of pain when all she needs
is a mouth guard to stop bruxism.
